12. If the applicant is a child:

If a child is only traveling with one parent, the written consent of the other parent or guardian is required, except in cases where the parent has the right to care or guardian alone. *

If a child is traveling alone (unaccompanied), the written consent of both parent or guardian is required to care for and guard the child. *

Visa applicant's birth certificate. *

Copies of identity cards for both parents. *

7. A copy of household registration book *

8. A copy of the Marriage Registration or Divorce Certificate (if applicable) *
